The Mental Health Reviews, Accommodation and Projects team works across both Richmond and Wandsworth. We are an established and diverse team of professionals and as a Social Care Assessor you will play a crucial role in assessing and reviewing the social care needs of adults with a mental health need.

Can you help us to make a difference to the lives of people approaching adult social care for the first time or who are already receiving support from us?

About the role

Reporting to the Senior Social Worker / Senior Occupational Therapist, you will work in a flexible way to assess or review the eligible social care needs of adults and carers with less complex needs to ensure that screening does not trigger the need for an assessment by a Social Worker or Occupational Therapist. You will carry out these assessments in accordance with the Care Act and input urgent care packages if required. 

You will play a key part in completing reviews to ensure that needs are being met while providing information and advice concerning Council services, health services, welfare benefits and access to local voluntary services.

Working with adults or carers to develop and agree support plans (including in an emergency), you will promote well-being by meeting eligible, unmet needs and make clear informed decisions about the level of support required; while ensuring these plans use resources to the best effect and that authorisation is provided where necessary.

Essential Qualifications, Skills and Experience

  • A willingness to work towards a social care qualification (e.g. NVQ II, III, etc.).
  • Demonstrate an understanding the Care Act 2014 and the roles and responsibilities within Adult Social Care Services including safeguarding.
  • Experience of working with clients / service users, preferably in social care.
  • Ability to communicate clearly and concisely; verbally and in writing, using appropriate IT.
  • Numeracy skills to support understanding of personal budgets and best value.
  • Ability to communicate with adults to identify their needs and to work with them to identify how these could be met.
